The SoulTaker: Tamashiigari

A human colony on the moon is destroyed by disease, prompting Kyosuke Date and his mother to escape to Earth, where they hide in a cathedral. His mother, concealing her identity as a nun, is killed after their location is discovered, stabbing Kyosuke before her death. He later awakens in the home of a girl named Runa, who claims to be his twin. Runa has also experienced a traumatic death at the hands of her adoptive mother, her soul fragmented into multiple "flicker girls" targeted by the Kirihara Group. Kyosuke survived due to his DNA's resistance to the disease, allowing him to transform into an alien mutant called the SoulTaker. He now fights against a hospital of mutants and corporate assassins to protect himself and Runa.
