A high school student named Kono Takaaki experiences his springtime days surrounded by several girls, including his childhood friends Yusuhara Konomi and Kosaka Tamaki. He interacts with a variety of girls, each with distinct personalities. Though the title suggests a connection to "To Heart," the two stories are unrelated beyond sharing the same school setting, with "To Heart 2" taking place two years later.
