Hanbun no Tsuki ga Noboru Sora

A seventeen-year-old boy named Yūichi Ezaki and a girl named Rika Akiba are both admitted to a hospital in Yūichi's hometown. Yūichi is being treated for hepatitis A, and Rika for a weak heart valve. As they spend time together, their relationship begins to develop. The setting is implied to be Ise, in Mie Prefecture, as shown by a sign in the first episode of the anime.
