A high school sophomore named Takatou Iori is heading to school to give a love letter to his classmate, Kusanagi Mikuru, when he is hit by a truck after saving a first-year elementary schoolgirl. He is revived by a doctor with unusual tendencies and wakes up as a female first-year elementary student. He now attends the same class as the girl he rescued.
