Human mutations known as "diclonii" are born with horns and telekinetic abilities in the form of invisible limbs called vectors. These abilities vary in strength and number between individuals, allowing some to block or deflect bullets. Diclonii display a high tendency for violence, which increases as they age, shifting from playful aggression in childhood to outright hatred of humans in adulthood. They are rare, as they are the only ones capable of natural reproduction. More common are "silpelits," born from human parents, where the father has been infected by the mutation. Only males can pass on the mutation, and all offspring are female. Scientists have observed a rise in horned infants committing violent acts, leading to their containment in a facility for experimentation or execution. Kouta, a non-mutant, becomes entangled with Lucy, the only pure diclonius, who seeks to destroy humanity. Lucy is devoid of empathy except for Kouta, while Director Kakuzawa, who has diclonius ancestry, aims to breed with her to start a new species. Their childhood connection is complex, and as an airborne virus spreads in Japan, the future of humanity remains uncertain.
