Mass comas strike the city, prompting Shirō and his companions to target the Masters and Servants located at the Ryudōji shrine. During this operation, they confirm that Sōichirō Kuzuki, a teacher of Shirō, is actually the Master of Caster. Following this revelation, Caster captures Sakura to use her as a sacrifice for summoning the Holy Grail. The group must confront these threats while dealing with the mysterious medical emergencies affecting the urban population.
