Black Jack reflects on Pinoko’s birth during her birthday celebration. Years prior, doctors brought a masked woman with a teratoma to him after other surgeons failed. Black Jack agreed to operate, hearing the tumor speak, and preserved its contents in culture. He constructed a synthetic body for the entity, naming her Pinoko and raising her with strict independence. A year later, the masked woman returned for a check-up. Upon seeing Pinoko, she rejected her as a sister, calling her a monster. Pinoko attacked the woman in distress, crying that she could never understand. The woman fled, leaving Black Jack to care for the devastated Pinoko.
