Sharaku frightens Pinoko with a horror tale, which proves relevant when a bandaged man visits Black Jack. The patient suffers from Dissociative Identity Disorder, where a violent personality emerged after his face distorted from a carbuncle. Black Jack performs surgery to restore his appearance, but the condition returns. A second operation successfully removes both the carbuncle and the alternate personality. However, when Black Jack arrives to collect payment, the man attempts to kill him. The doctor and Pinoko must find a way to escape the crazed patient.
