Black Jack agrees to impersonate a patient's son to perform surgery on his ailing elder sibling. During the waiting period, he learns that the older brother sacrificed his own dreams to fund the younger brother's medical education. The younger brother fled the pressure and built a business empire, yet his son is currently studying medicine in America. Black Jack delays the procedure for a week to prepare. When the surgery begins, the skeptical elder brother realizes the surgeon is not his nephew and refuses treatment. However, Black Jack had previously traveled to New York to retrieve the actual nephew. The operation ultimately succeeds after the estranged brothers reconcile and accept the true identity of the surgeon.
