A nurse informs Conan that Eisuke underwent leukemia surgery in his youth, implying Rena might be his sister. Simultaneously, the FBI narrows their suspect list to three patients: an elderly man with a broken leg, a man suffering from a neck sprain, and another with dorsalgia. Conan offers to secretly investigate these individuals. He records footage of the three patients using a hidden camera and subsequently reports his findings back to the FBI agents.
