Ep. 3: Sayaka visits Kyousuke while Mami warns Homura before a fatal witch encounter.
Sayaka visits injured violinist Kyousuke Kamijou, whom she admired in childhood. Mami reveals she suffered a traffic accident and lacked time to consider her wish, noting precedents for wishing on others' behalf. That night, Homura warns Mami against making Madoka a magical girl. The following day, Madoka and Sayaka discover an activating Grief Seed. Sayaka and Kyubey enter the portal while Madoka finds Mami. Homura attempts to warn Mami, but Mami binds her to prevent interference. Madoka expresses her desire to become a magical girl to help others, which makes Mami happy despite her loneliness. When the Grief Seed activates, Mami fights the witch, which transforms into a giant caterpillar that decapitates and kills her instantly. Homura defeats the witch and claims the Grief Seed, leaving Madoka mortified and Sayaka resentful.
