Ep. 15: Nadeko Medusa Part Four details the aftermath of Nadeko swallowing the talisman and defeating Koyomi and Shinobu.

After Nadeko consumes the talisman and revives Kuchinawa, she confronts Koyomi and Shinobu at an abandoned temple, where she defeats them. Kuchinawa explains that their prior interactions were imaginary, stemming from Nadeko swallowing the talisman. Ougi provided the talisman's location, and Nadeko’s actions were a scheme to infiltrate Koyomi’s home for a key. Before killing Koyomi, Nadeko receives a call from Hitagi, who demands she kill Koyomi, Shinobu, and herself to avoid being killed by them. Nadeko agrees to return on graduation day. Six months later, on graduation day, Nadeko waits at the polar snake shrine. Koyomi, Hitagi, and Shinobu arrive accompanied by Suruga and Tsubasa to confront her.
