After Mireille and Kirika assassinate a prominent Cosa Nostra member, the Mafia recruits Lady Silvana Greone, known as the Intoccabile, from Sicily to eliminate Noir. Silvana lures them to a forest cottage, accompanied by three elite Sicilian assassins called the Three Mafia Saints. Kirika engages Paolo and Dominic outside the building, while Mireille enters to kill Francesco. Despite her success, Mireille struggles to confront Silvana, who has been a source of fear for her since their childhood meeting. The encounter highlights the deep personal history between Mireille and Silvana, complicating the battle against the formidable Sicilian leader and her skilled henchmen in this deadly confrontation.
