Tsunade identifies an insect bomb inside the Raikage after Ay’s group is ambushed by the Nokizaru. She destroys the spy bug and prepares for surgery, assigning Shizune to lead while she supervises. Although Ay protests, believing Tsunade should operate, he is reassured. During the procedure, blood splatters on a terrified Tsunade, causing her to leave. Shizune explains Tsunade’s past losses to Ay, who argues she is unfit. An indignant Shizune throws the extracted bug at his feet. The scene concludes with Naruto and Bee heading to the battlefield, while the Raikage and Tsunade return to headquarters.
