Black Jack prepares to return to Japan with Pinoko but is summoned to the Sky Hospital. There, he finds Zen Mantoku, the sole patient suffering from the Phoenix disease due to Noir Project B.O.P. injections. Jack agrees to help Dr. White create a cure for 1¥ billion. Meanwhile, Renka, bitter over losing her husband and daughter and resentful that Jack gave her her husband's ex-wife's face, secretly plots to kill Jack and steal the vaccine to profit from spreading the disease. During this tension, a henchman attempts to hijack the hospital, causing Mantoku to escape his containment capsule and release a mutated strain of the Phoenix disease aboard the aircraft.
