Boccherini reports a miraculous recovery, but Black Jack suspects the outcome is too radical. The boy subsequently dies. Enraged, the billionaire arranges for the medical association head’s son to be shot. The father pleads with Black Jack to save the boy. During this tension, a henchman attempts to hijack the hospital, causing Mantoku to escape his containment capsule and release a mutated strain of the Phoenix disease aboard the aircraft.

Renka attempts to assassinate Jack but is stopped by her father, Zen Mantoku, resulting in both being shot. Zen dies, but Jack operates to save Renka, discovering an anti-virus in her system that can cure the disease. However, the missile has already been launched. As the disease spreads in Black Jack’s body, he faints and sees visions of Benitokage, Dr. Kiriko, Dr. Honma, his parents, and an adult 18-year-old Pinoko, who urges him to continue. Dr. White successfully evades the missile detonation, ensuring everyone’s safety. The cure, named Honma Antiserum after Dr. Honma, is made freely available. Black Jack and Pinoko return to the site of his ruined house to rebuild.
